CAWES Battle of the Bands Participant Waiver and Release Form

Congratulations on being selected to participate in Battle of the Bands: Voices for Change. As part of your participation, each band is required to complete a Participant Waiver & Release Form on behalf of all of its members. Please ensure this form is completed in full prior to June 15, 2026.

Band Submission Disclosure

This form is to be completed once per band. By submitting this form, you confirm that you are completing it on behalf of all members of your band. You are responsible for ensuring that all band members are aware of and agree to the terms outlined in this waiver and release.

Assumption of Risk

We understand that participation in a live music event may involve risks including, but not limited to, personal injury, hearing damage, equipment-related injury, and loss or damage of personal property. We voluntarily assume all risks associated with our participation.

Release of Liability

We release and hold harmless Central Alberta Women’s Emergency Shelter (CAWES), event organizers, sponsors, venue partners, staff, volunteers, and affiliates from any liability, claims, or damages arising from participation in this event.

Medical Responsibility

We confirm we are physically able to participate and are responsible for our own medical coverage and expenses.

Equipment and Property

We understand We are responsible for our own instruments, equipment, and personal belongings

Code of Conduct

We have read the event Rules and Guidelines and agree to behave respectfully and professionally. We understand that inappropriate behaviour may result in remove from the event and band disqualification.

Media Release

We give permission for photos, video, and audio recordings of our performance and participation to be used for promotional and marketing purposes without compensation.

Rules of Agreement

We agree to follow all event rules and understand that failure to comply may result in disqualification or removal.
